When the person you are caring for leaves hospital they should be provided with personalised information about their stroke and recovery. This is often a paper booklet, sometimes called a Personalised Stroke Record. This should provide information about the type of stroke they have had, how this may affect them and their personalised goals for recovery. If the stroke survivor has not received anything, ask a member of your stroke team if you can have one. There are some useful links below, about the transition home:
- Healthwatch UK – advocate for patients and carers to ensure your experiences help to inform improvement to health and care services. Find your local HealthWatch.
- Mid Yorkshire NHS Trust - offer support to any carer who has a loved one admitted to Pinderfields, Pontefract or Dewsbury Hospitals. You can attend discharge meetings or other key meetings to ensure you and your loved ones get the care and support you need. There is also a Carers Passport & Carer Lanyard to help identify carers on the wards or at outpatient appointments. The hospital support workers can be contacted on 01924 305544.
